Every time you go to a new providers office there are a ton of forms to complete and it delays your personal schedule. If you follow the links here you can download and fill out all those pesky forms in advance so when you arrive in the office a short registration time and you are ready to be seen.

Consent to treatment - if you have are signing as an adult it is ok to just draw a line through the blanks mentioning children. IF you are a parent or guardian fill in the dependents name on each line.

Patient History - All good stories start with once upon a time.... In healthcare that is called the patient history and it is usually the form that takes the longest to complete. You can complete it on line (mostly) or by hand. Just remember to bring it with you or "Those who don't remember history will have to repeat history."

Controlled substances contract - we do utilize the Nevada Controlled Substance Task Force database before prescribing controlled substances. We retain the right, and exercise the responsibility, to determine when to provide controlled substances for a health care condition.

Privacy notice - Complies with  HIPAA requirements read online then complete the signature sheet and bring it with you. If you want a copy of the privacy statement for your records feel free to print it out.

Records request - If you are transferring from another practice we may need to obtain your records to insure we are providing continuity of care. Please complete and print a form for each provider from whom you would like us to obtain records.
